• Even if your Insurance/TPA company is not empaneled at Adiyogi Hospital. You can still apply for cashless insurance.
• Policy holder will send planned hospitalization information to their company.
• For this patient has to contact their TPA/Insurance company and drop a mail to them with CC to Adiyogi Hospital. Mail ID : adiyogihospitaltpa@gmail.com
• Patient has to attach the below documents in the mail
o Adhaar card (Front and back)
o Policy copy
o OPD prescription
o All necessary investigations :X-ray, USG,Blood reports etc..
• Patient has to mentioned the date of surgery along with the Doctors and Adiyogi Hospital name.
• Need to inform at-least 72 working hours before surgery
• The charges provided us have to be accepted/mutually agreed upon by the insurance company to proceed for the treatment.
